Only fully completed applications will be accepted. Do not send incomplete forms. Scanned forms are acceptable.

  • You must download, complete, and return to us, a copy of our application form. After completing the form, return it, along with your completed signature page, signed rules page, a copy of your health card and your deposit payment to:

  • The cost of the trip will be $3400 which will include transportation, housing, food (breakfast & supper) and admission to scheduled attractions.

    For spending money we suggest credit card, debit cards for security purposes.

    A $1400 deposit must accompany this application. Full payment is due by April 1, 2018.

    Send a personal check, cashier’s check or money order, made payable to:
    USA Tour. No credit cards, cash, International checks or International money orders accepted. Checks must be drawn on an American Bank.

    On the payments page, you can use paypal to pay with credit cards or with your own paypal account. Additional charges are included in "buy now" payment amount.

    In the event that we do not have 48 students registered by March 15, 2018 we reserve the right to cancel the trip and return all monies paid. Only computer generated or typed forms are acceptable.
